CHADIS is an online system that allows you to receive and complete the developmental questionnaires at home before your visit using your portal account. We will send you the questionnaires a week before your appointment. You can use your phone, tablet or computer to complete them. When you are done, they are submitted directly to your child’s chart for your provider to review.
