The summer months mean we are receiving college, sports and camp forms to complete for our patients! If your child has had a well check in the past 365 days, we are able to complete most forms (some require a well check in the past 6 months) based on that visit.  Drop your form off at the office or upload a .pdf to the portal. If you need a well check, we have openings in each office – Please give us a call to schedule!  We can complete your form at that visit. Please remember we ask for 7-10 business days to complete forms. Once the form is complete, we upload to the portal or you can pick up at the office 10 days after you drop it off.  Due to the volume of forms received, we do not call when it is ready. Thank you!
